How to Refer Patients:
Referral forms and medical records can be faxed to (417) 890-6151. Insurance information is not needed for referrals.
After receiving a referral form, we will get in touch with the patient to conduct a phone pre-screening. If they meet the initial requirements for the study, we will schedule a screening visit, which may involve additional testing to determine study eligibility. Once the patient is enrolled and provided their approval, we will share all test results and relevant study information with you.
